Internship Contract
The Internship Contract is the most important internship document. This is the agreement made between the intern, the host company, and the Queens University of Charlotte Internships Office. The Internship Contract must be completed in its entirety, including signatures, by all parties involved. Once the Internship Contract has been completed, it will be held on file in the Internship Office. Please make sure you have the most recent version as revisions were made in 11/07. Work Hours Log
All interns are required to maintain a log of the hours they have worked on-site at their internship. The form must be completed, signed by the intern and verified by the on-site internship supervisor. All Work Hours Logs are to be turned in to the Internships Office at the end of the semester. Interns are required to work a minimum of 86 work hours per 2 academic credits.

INTE 497 Work Hours Log (for excess credit hours or nonrequired internships)
Hayworth Internship (Excess Credit) Application Form
Hayworth students applying for more than 3 credit hours per semester or for a second internship must complete this form with their department head from their major of study and submit it for review to Andrea Alford, manager of internships, before registering for INTE 496. This form is to promote academic internships that are suitable to replace elective credits in your major. It is a MS Word document that you can fill out online and print out for signatures.
