To be ready for your internship you will need to fill out the Internship Site Contact List Form and have this document completed when you initially meet with your Internship Coordinator. Download this form, fill it out and schedule a meeting with you Internship Coordinator and you are on your way toward a great internship oppertunity.
After you complete your internship we want to hear about your experience. Please print the Internship Assessment Form and follow the instructions provided on the document.
If you are a PGM site internship supervisor you will need the Internship Evaluation Form to complete the requirements assigned to the student. Please send the Evaluation Form to one of the internship coordinators or to Eric Baldwin.
