Verification Worksheets 2020-2021
While some requirements vary, all students selected for verification must complete a Verification Form. Please review your notification carefully to determine which of the forms you are required to complete and submit as part of your total documentation requirements. There are two verification forms to choose from - one for dependent students and one for independent students.
The notification you received from Queens advising you that you were selected for verification indicates whether you need the dependent or independent student version of the verification form. In addition, your SAR indicates whether the federal government considered you a dependent or independent student. Your notification from Queens will also tell you which verification form you need to complete.
In addition to a verification form, you may be required to complete an Identity/Statement of Education Purpose and Verification of High School Completion form. If you are required to complete this form, you will receive a notification of this requirement from Queens. A link to this form is shown below.

Important Information about Verification for 2020-2021
The Federal Government selects a certain number of Free Application for Federal Student Aid (FAFSA) applicants for a process called 'verification'. For those students whose FAFSA is selected, the school is required to verify some or all of the information provided on the FAFSA.
If a FAFSA is flagged for verification by the government, the student will be notified on the Student Aid Report (SAR) received in response to the original FAFSA filing. The school is also notified. All students selected for verification will receive a notification from Queens advising them that we have received their FAFSA information and that they have been selected. The notification will indicate the documentation needed for us to complete the verification process.
The verification process is designed to verify all income and basic household information to ensure the Expected Family Contribution (EFC), determined from information provided on the FAFSA, is correct. This is important because the EFC determines the amount of need-based federal and state aid a student receives. The EFC is also used to determine how much institutional need-based aid might be awarded. For this reason, all federal and state aid, and institutional need-based aid, is an estimate only, and not certain, until the verification process is completed.
Please note that until the verification process is completed and we can confirm the student's eligibility for aid. Additionally, no aid will be disbursed to the student account. For students admitted or continuing for the fall term, if the verification process is not completed by October 15, we will cancel all federal and state aid (including loans). The student will also forfeit any Queens need-based funds that may have been awarded, whether or not the verification process is completed after this date. For students admitted for the spring term, the process must be completed by February 15.
For federal Pell Grant eligible students whose application has been selected for verification, verification must be complete by a deadline published annually in the Federal Register (generally no later than 120 days after the last day of enrollment during the academic year) or the Pell award will be lost. For all other federal aid, verification must be completed one week before the last day of enrollment during the academic year. Failure to complete verification will render the student ineligible for need-based aid, including loans, for that academic year.
While state aid has essentially the same deadlines as federal aid, it is awarded state-wide on a first come, first served basis. This means that a student can lose access to state aid if verification is not completed within the first few weeks of the fall term.
If a FAFSA transaction is selected for verification after an award has already been made and/or disbursed and requested verification documents are not provided, any undisbursed award will be canceled. No additional federal, state or institution need-based funds will be disbursed until verification has been completed.
Additional Information and Documentation Requirements
Documentation requirements necessary to complete the verification process differ from student to student. The notification is emailed advising that the student's application was selected for verification included a list of documents we need to receive. In addition, the student's MyFinancialAid portal has the complete list of documents together with an indication as to whether they have been received in the Student Financial Services Office.
For 2020-2021, information to be verified includes number of household members; number of household members (excluding parents) who will be enrolled at least half-time in a degree, diploma or certificate program at a post-secondary educational institution any time between July 1, 2020 and June 30, 2021. For 2018 federal income tax return filers, additional income-related information to be verified includes adjusted gross income, U.S. income tax paid, untaxed IRA distributions, untaxed pensions, education credits, IRA deductions, and tax-exempt interest income. Income earned from work must be verified for non-filers of U.S. income tax returns. In addition, other sources of untaxed income may also need to be verified.
Income-related information for U.S. income tax return filers is verified either through the FAFSA IRS data retrieval process or through an official IRS tax return transcript.
To complete the IRS data retrieval (if you did not do so when you first filed your FAFSA), go to www.fafsa.gov, log in to your FAFSA record, select "Make FAFSA Corrections", and navigate to the Financial Information section of the form. From there, follow the instructions to determine if you are eligible to use the IRS Data Retrieval Tool (DRT) to transfer 2018 IRS income tax information into your FAFSA.
Financial Aid applicants who are not able to use the IRS Data Retrieval Tool may request a free 2018 IRS tax return transcript by calling 800-908-9946, or by filing IRS form 4506-T.
If verification causes a change in the student's eligibility for federal, state or institutional need-based funds, the student will be notified that their financial aid package has changed.
